Kiara Terrell
RN, BSN is a Florida Native, who relocated to Houston, Texas in 2018 with hopes of advancing her nursing career in the city known for its “ Medical Center ”. Kiara has been employed in centralized hospital float pools, nurse travel agencies and as well local nurse staffing agencies . This has allotted Kiara the opportunity to be trained and assigned to work within many specialties and develop a vast knowledge base for much of the body of nursing and how it operates in healthcare.
Kiara has worked in long term care,assisted living, home health pediatrics, LTAC, rehabilitation hospitals, substance abuse and behavioral health, trauma hospitals, IMU, ED, Med Surg Tele, Ambulatory surgery centers, hospice, hematology/oncology and endoscopy and all since becoming a nurse in 2013.
Kiara noticed a remarkable trend with patients and their health, and it revealed that they weren’t knowledgeable of their diseases and how to properly manage their own healthcare . Patients often didn’t know the names of any medications they were prescribed, why they were taking it, or non compliant for many different reasons. Medication mismanagement often led to decline and unnecessary hospitalizations.
Patients were left untreated for lack of knowledge for proper resources. Patients just didn’t understand why and how they even got sick in the first place and how to improve their quality of life. Operating at the bedside as a nurse, Kiara learned that her presence during those 12 hours were many patient’s saving grace as she was their educator, advocate, and interventionist.
This was the biggest reason why she remained a bedside nurse for so long before pursuing her master’s as a Family Nurse Practitioner, she feared taking their voice away. What Centered Care offers as a company is an extension of processes she mastered within her own career as a nurse before CMS.gov implemented these programs.
CMS made a statement stating “We acknowledged that the care coordination included in services such as office visits does not always describe adequately the non-face-to-face care management work involved in primary care and may not reflect all the services and resources required to furnish comprehensive, coordinated care management for certain categories of beneficiaries”
Management
This statement is not only true for private practices but for all of the healthcare delivery systems. All members of the healthcare team deliver services outside of what is actually being billed, and in most cases patients are cut short of the time needed because a typical work shift doesn’t allow all patients to have the luxury of the amount of time it typically requires to adequately counsel them.
So Kiara started this company with the intention of bridging the gap between the patient and their health care disparities using evidence based practice, previous nursing experience and the laws that govern these practices through the CMS. She knows the value of time within the healthcare team as well as how limited that time can be with the call of demand for them. Kiara is so grateful to implement Centered Care to reach back to support her healthcare family while continuing to improve population health.
